Diabetes Diaries Contest
2011 OFFICIAL RULES
HOW IT WORKS:
Beginning on Monday, December 19th, Ohana Media Group Anchorage, Coastal
Television Broadcasting Co. LLC and additional sponsors will encourage individuals to
write an essay (one-hundred and fifty words or less) that explain how diabetes has
touched his/her life and/or what lifestyle changes he/she has made in the steps towards
prevention at 921bob.fm, kfat929.com, 963thewolf.com, 1021thepeak.com, kmbq.com,
or kbyr.com. After the Sunday, January 29th deadline, Ohana Media Group, Coastal
Television Broadcasting Co. LLC, and participating sponsors will choose a first and
second place winner based on essay message.
Your essay must be one-hundred and fifty (150) or less words in length, or it will be
disqualified. Ohana Media Group or Coastal Television Broadcasting Co. LLC reserves
the right to add the Diabetes Diaries logo where applicable. All submitted essays must be
approved by Ohana Media Group and Coastal Television Broadcasting Co. LLC before
they are available for public viewing. You may enter as many essays as you like. Each
entrant must designate one contact person who must have at least one (1) valid email
account in order to qualify. By entering, you agree to be bound by the decisions of the
judges, to be bound by these Official Rules, and to comply with all applicable federal,
state, and local laws and regulations. Ohana Media Group and Coastal Television
Broadcasting Co. LLC (and their affiliates) or sponsors and those working for Ohana
Media Group and Coastal Television Broadcasting Co. LLC or sponsors or on behalf of
Ohana Media Group and Coastal Television Broadcasting Co. LLC or sponsors, will not
be responsible for lost, late, misdirected, damaged, or postage due mail or e-mail, or for
Internet, computer hardware and software, phone, and/or any other technical errors,
malfunctions, or delays. Entries which are incomplete, inaccurate, forged, irregular in any
way, or otherwise not in compliance with these Official Rules will be disqualified. In the
event of a dispute concerning who submitted an entry, the entry will be deemed to have
been submitted by the authorized holder of the e-mail account from which the entry is
made. The "authorized account holder" is the natural person to whom an email address is
assigned by an Internet access provider, online service provider or other organization
(e.g., business, educational institution, etc.) responsible for assigning email addresses for
the domain associated with the submitted email address.
Each submission must be original and created entirely by entrant. Submissions may
include materials which are in the public domain or for which you have obtained the prior
written permission or clearance from the owner. Submissions must not incorporate or
include anything that violates any law or the copyright, trademark, publicity right,
privacy right or any other right of any third party. If you become a Finalist or Prize
Winner, Ohana Media Group and Coastal Television Broadcasting Co. LLC may require
sufficient written documentation to establish that you have properly obtained lawful
permission to incorporate names, likenesses, identities, persons or materials used in your
submission or that the same are in the public domain. Ohana Media Group and Coastal
Television Broadcasting Co. LLC may, at their sole and exclusive discretion, disqualify
any submission it believes incorporates names, likenesses, identities, persons or materials
without permission or are not otherwise in the public domain.
Entrants will be able to enter their one-hundred and fifty word or less essays from
December 19, 2011 through January 29, 2012. Entries will be judged by a panel of
judges (one judge from the participating sponsor, American Diabetes Association-
Alaska, and one judge from Ohana Media Group and Coastal Television Broadcasting
Co. LLC – judges are subject to change). Judges will base their decision on the criteria of
originality, creativity, concept, information and overall presentation. The decision of the
judges regarding the grand prize winner and runner-up shall be final and binding in all
regards.

PRIZES:
· Grand Prize: a cash prize of $1,000 and your entry aired on Coastal Television
Broadcasting Co. LLC and on all 6 Ohana Media Group radio stations (KBBOFM,
KFAT-FM, KXLW-FM, KDBZ-FM, KMBQ-FM, and KBYR-AM) and all 6
radio station websites.
· Second Prize: a cash prize of $500 and your entry aired on Coastal Television
Broadcasting Co. LLC and on all 6 Ohana Media Group radio stations (KBBOFM,
KFAT-FM, KXLW-FM, KDBZ-FM, KMBQ-FM, and KBYR-AM) and all 6
radio station websites.
THE FOLLOWING GENERAL RULES APPLY:
1. No purchase is necessary to enter. Contest is subject to federal, state and local laws and
is void where prohibited by law.
2. Eligibility and Limitations. Participants and winner(s) must be U.S. citizens and
residents of Alaska. Military entrants with out of state IDs must establish proof of
residence in the state of Alaska to be considered. All contest winners must disclose his or
her legal name, address, telephone number, date of birth and Social Security Number.
Any winner found to have supplied false or misleading information may be disqualified
from the contest and therefore forfeit the prize(s). Age restrictions may apply. All
entrants to any contest both imply and agree to participate in the contest and obey the
rules of the contest. If two or more persons enter, or a company enters, the group of
persons or company must designate one individual as its or their contact. Only legal U.S.
residents at least 18 years of are eligible to enter the Contest. The only persons, groups of
persons or companies excluded from participating in the Contest are the Employees of
Ohana Media Group or Coastal Television Broadcasting Co. LLC and any and all parent,
subsidiary, related and affiliated companies, and their respective officers, directors,
employees, and agents and their dependents and immediate families, as well as
employees of contest sponsors and vendors, and employees of other broadcast stations or
media outlet are not eligible to participate or win. If you are entering on behalf of a group
of persons or an entity, you understand and acknowledge that use of the words “you” or
“your” in these Official Rules refers to the group of persons or entity on whose behalf
you have entered (unless the context indicates otherwise).
3. Release of Liability & Identification of Participants/Use of Voice and Likeness. Ohana
Media Group and Coastal Television Broadcasting Co. LLC and any other contest
sponsors and their affiliated companies and their respective officers, directors and
employees shall be released and discharged from any and all legal claims, losses injuries,
demands, damages, actions, and /or causes of actions which arise out of and/or are in any
way related to the prize(s), the receipt, use and/or enjoyment of the prize(s) and/or
damages or losses related to the contest. If a Sponsor pulls out of a contest for reasons
beyond Ohana Media Group or Coastal Television Broadcasting Co. LLC’s control,
Ohana Media Group or Coastal Television Broadcasting Co. LLC are not responsible for
continuing the contest and may substitute prizes to fulfill obligations. Listeners
understand that if they call-in or enter any Ohana Media Group or Coastal Television
Broadcasting Co. LLC contest (mail-in, web site or on-air) they may be called by Ohana
Media Group or Coastal Television Broadcasting Co. LLC personnel live on-the-air, or
taped live-on-the-air for use as part of the on-air contest or in future advertising on any
Ohana Media Group or Coastal Television Broadcasting Co. LLC broadcast station.
Listeners further understand that permission to use their voice, likeness and/or name is
granted by entering to win or calling-in to win any contest on an Ohana Media Group or
Coastal Television Broadcasting Co. LLC broadcast station. Each winner further grants
Ohana Media Group, Coastal Television Broadcasting Co. LLC, and their sponsors and
its affiliates full permission and authority to use his or her name, likeness and/or voice in
all forms of advertising without further compensation or permission. (a) Entries and other
submitted material become the property of Ohana Media Group Anchorage and Coastal
Television Broadcasting Co. LLC and will not be acknowledged or returned. By
submitting an entry: (i) You represent and warrant that, if you are the designated person
for a group of persons or company that has entered this Contest, you are authorized to
make representations and warranties on behalf of the group or the company, and the
company or you and the group of persons, as the case may be, will be jointly and
severally liable for any breach of these representations and warranties or other failure to
comply with the Official Rules of this Contest.(ii) You represent and warrant that your
entry and all elements contained therein (“Content”) is original and, to the extent the
Content incorporates or includes anything that is owned or controlled by any third party,
you have obtained in writing all rights necessary to comply with the Official Rules of this
Contest; and, that Content does not violate any copyright, trademark, publicity right, or
any other right of any third party.(iii) You agree that content shall be, to the maximum
extent eligible, a "work made for hire," under the U.S. Copyright Act, as amended, with
all rights therein, including without limitation the exclusive copyright, being the property
of Ohana Media Group Anchorage and Coastal Television Broadcasting Co. LLC.
Otherwise, if your entry or any element thereof is considered not to be a "work made for
hire," you hereby unconditionally and irrevocably transfer to Ohana Media Group Ohana
Media Group Anchorage and Coastal Television Broadcasting Co. LLC all right, title,
and interest in the Content (including, without limitation, the copyright) in any and all
media whether now known or hereafter devised, in perpetuity, wherever produced or
located, including the right to use, copy, distribute, perform, display and to create
derivative works of the Content for advertising, trade, other commercial purposes or any
other purpose; provided, however, to the extent Content contains third party materials,
you have obtained written permission granting Ohana Media Group Anchorage and
Coastal Television Broadcasting Co. LLC a perpetual, royalty-free, fully paid, freely
transferable (with full rights to sublicense) universal, unconditional and irrevocable right
and license to copy, distribute, perform, display and to create derivative works from such
third party materials for any and all purposes relating to the promotion, marketing,
advertising, distribution and/or sales of Ohana Media Group Anchorage and Coastal
Television Broadcasting Co. LLC or other third party products and services for cancer
and cancer screening. (iv) You represent and warrant that you meet the requirements and
qualifications for this Contest, and that you have read these Official Rules and are fully
familiar with them. (b) You agree (and if you are the designated contact person for a
group of persons or a company, agree as an authorized agent on behalf of that group of
persons or company) to indemnify, and hold Ohana Media Group Anchorage and Coastal
Television Broadcasting Co. LLC, their officers, directors, employees, contractors, agents
and representatives (“Indemnitees”) harmless from and against any third party claim
(including reasonably attorneys’ fees and costs) arising from any use of the Content. You
waive (i) any right to publicity, privacy or moral rights relating to the Content or your
participation in this Contest, and (ii) any right to inspect or approve uses of the Content
or to be compensated for any such uses. To the extent these rights may not be waived
legally, you agree not to assert them.
4. Telephone Contests. If a contest is a call in to win contest and/or a telephone is needed
to participate in the contest, participants are restricted to the use of ordinary telephone
equipment. Participants that enable the “Caller ID” block function will not be allowed to
participate unless they enter their correct area code and telephone number if prompted, or
disable the features inhibiting their participation in the contest. No liability is assumed by
Ohana Media Group or Coastal Television Broadcasting Co. LLC for faulty,
mistranscribed, misdirected/misdialed telephone communication or technical difficulties
experienced by callers throughout the call-in portion of the contest including: failed,
incomplete, garbled or delayed transmission of online entries, traffic congestion on
telephone lines, the Internet or at any website or lost or unavailable network connections
which may limit an online entrant's ability to participate in the contest, and any injury or
damage to entrant’s or any other person’s computer related to or resulting from
participating in or downloading any information necessary to participate in the contest.
Poorly connected or disconnected cellular phone calls may be disqualified. Calls for
contestants are taken in order of receipt. In the event that more than one voice is heard on
the phone line, the contestant/qualifier or winner will be the first person to speak. If the
judge(s) is not able to determine which caller was first, a random drawing will be held to
determine the winner. The use of switching or programming equipment by telephone
company employees is in violation of contest rules and is not permitted. Telephone
contests with “cues to call” are only official when heard on-air via radio. Due to delays in
audio transmission, internet streaming is not a valid contest option.
5. Mail/Fax/Text Contests. Only one entry per person will be accepted unless otherwise
specified on-the-air or in separate contest rules. Winning entry(s) will be drawn at
random from all valid entries received. Ohana Media Group and Coastal Television
Broadcasting Co. LLC are not responsible for late, misdirected, lost or mutilated mail,
entry forms or faxes. No liability is assumed for any malfunction of computer equipment,
fax machine equipment or fax machine telephone lines. If entry is illegible or does not
supply all information deemed necessary to declare a winner, entry will be deemed
invalid and another entry will be selected. Standard texting fees will apply to any text
contest entered.
6. Entry Form Contests. Contestant must complete an official entry form and deposit it in
the provided entry box. Only one entry per person may be submitted for each contest.
Winning entry(s) will be drawn from all valid entries received. If an entry is deemed
illegible or does not supply all the information necessary to declare a winner, the entry
will be deemed invalid and another entry will be selected.
7. Website Contests. To enter a contest on a Ohana Media Group or Coastal Television
Broadcasting Co. LLC stations’ website, contestants must complete all the information
deemed necessary to declare a winner. No liability is assumed by the Ohana Media
Group or Coastal Television Broadcasting Co. LLC for any malfunction of the internet or
individual computer systems. Only one entry per person may be submitted. Winning
entries will be randomly selected from all valid entries received. If entry does not supply
all the information deemed necessary to declare a winner, entry will be deemed invalid
and another entry will be selected. When entering a contest via an advertiser’s web
banner or logo hyperlink, Ohana Media Group or Coastal Television Broadcasting Co.
LLC is not responsible for prize fulfillment or any aspect of the contest.
8. Determination of Winner(s). Final determination of all winners will be made solely by
Ohana Media Group, Coastal Television Broadcasting Co. LLC, and any sponsor judges.
Any and all decisions made by the Ohana Media Group and Coastal Television
Broadcasting Co. LLC staff and sponsor judges are final. Contest rules are available by
request at Ohana Media Group’ or Coastal Television Broadcasting Co. LLC’s main
office buildings. Ohana Media Group main address is 833 Gambell Street, Anchorage,
AK 99501 and Coastal Television Broadcasting Co. LLC’s main address is 2700 E.
Tudor Rd. Anchorage, Alaska 99507. Official rules will also be posted online at

9. Transferability/Substitution of Prize. Prizes may not be traded or transferred.
Substitutions of cash or equivalent, unless otherwise specifically stated in the contest
rules are not allowed other than a substitution that may be necessary due to the
unavailability of a prize. If concert, other ticketed event, gift certificate or coupon has
been awarded as a prize and the concert, event, place of business, or product is
postponed, rained out, cancelled or for other reasons, beyond our control, does not
occur/exist, Ohana Media Group or Coastal Television Broadcasting Co. LLC will not be
responsible for replacing the prize. Prizes not redeemed by expiration date are no longer
valid and will not be replaced by station. In accepting a prize, the winner accepts to take
the prize as outlined in the contest. Dates for concert, flight departure/arrival, weekend
stays, etc, cannot be changed by winner. If a changing the dates is an option, all
additional expenses, change fees, etc. will be the responsibility of the winner. If winner is
not able or chooses not to accept prize “as is”, winner forfeits his/her prize. Ohana Media
Group and Coastal Television Broadcasting Co. LLC reserve the right to select another
winner. Winner is responsible for all costs associated with travel to any venue or event
for which a prize was awarded, unless otherwise specified. If the prize includes a trip,
winner agrees to accept all blackout dates, space availability requirements, etc.
established by Ohana Media Group’ and Coastal Television Broadcasting Co. LLC’s
prize partner(s), such as hotels, airlines, trains, ships, etc. Winner agrees that acceptance
of any trip taken as a prize from a Ohana Media Group station does so entirely upon their
own initiative, risk, and responsibility and winner and any travel companions will be
required to sign a Travel Release and form. If the trip requires traveling outside of the
United States, the winner, upon claiming the prize, must have a valid US passport. If the
winner does not have a valid US passport upon winning the prize, the trip may be
forfeited.
10. Requirements for Claiming Prize(s). Winner is required to complete an IRS Form W-
9 and a Winner Affidavit of Eligibility and Release form before prize will be awarded.
The W-9 and Winner Release will be sent to winner via mail, fax or email. Winner must
bring the completed paperwork when claiming prize, or return it by fax/mail before prize
will be awarded. Winners must return forms within two weeks of being notified that they
have won a prize. Winners can select one of two ways to receive prizes after completed
paperwork is received: A) Prize will be mailed via the United States Postal Service. A
parent or guardian must sign the release form if the winner is under age 18. Ohana Media
Group or Coastal Television Broadcasting Co. LLC is not responsible for prizes that are
lost, stolen, misdirected, or damaged; or B) Prize may be picked up in person during
regular business hours (8am to 5pm, Monday – Friday) at Ohana Media Group (833
Gambell Street, Anchorage, AK 99501).
11. Taxes and Expenses. The winner (or winner’s guardian) agrees to pay all expenses,
taxes & fees not included in the prize. Ohana Media Group will report to the government
any winnings totaling over $600 (in cash or fair market value) during the calendar year.
Winners will receive an IRS 1099 Tax form if any winnings are reported. Winners are
responsible for payment of all taxes on prize(s).
12. Amendments to Rules. Ohana Media Group or Coastal Television Broadcasting Co.
LLC reserves the right to amend the contest dates and rules at any time. If any portion of
the above rules is determined to be unenforceable due to local or state laws, all other
portions of these official rules remain in effect.
13. Postponement or Cancellation of Contest. Due to news events of the day, or any
reason beyond Ohana Media Group’ or Coastal Television Broadcasting Co. LLC’s
control, Ohana Media Group and Coastal Television Broadcasting Co. LLC reserves the
right to post-pone or cancel a contest without notice. In such instances, the contest prize
may be awarded at a future date at Ohana Media Group’ and Coastal Television
Broadcasting Co. LLC’s discretion.
